License renewals take place every two years. Paper renewal notices are no longer being sent out, so it is imperative that you have the correct e-mail address on record with the Department.
Your license must be renewed by October 31st of the odd-numbered year following your initial licensure. The renewal period is open one month before the expiration date and requires a $25 fee. Your supervisor will have to complete a Continuing Education Sponsor Questionnaire before you can begin your CE hours and send it to:

The sponsor allowing you to complete the CE hours is responsible for issuing a Certification of Attendance, so you can send it along with your application as proof the required hours have been completed. The certificate must contain the sponsor’s name, address, and license number. Your name and address must also be included with a short statement regarding the subject matter studied.
The hours each program was attended, as well as the date and location of the program, must be signed by the sponsor. You will be notified in writing if you aren’t meeting the CE requirements, and if the Board sees fit, you may face disciplinary action.
